- HOUSEHOLD 1 – Contact information for parents/guardians residing at the same address.
- HOUSEHOLD 2 – Please complete the household 2 section of the form for any other households who should receive information from Long Trail School.
BUSINESS/PROFESSIONAL INFORMATION – We encourage you to complete the additional business/professional section as well. Business information will be used to aid in our new internship program and utilized for guest speakers. Families may choose to purchase a business ad or listing to offset publishing costs of the Family Directory.
INTERESTS/HOBBIES – If you wish, please complete the interests/hobbies section. The Long Trail Family Network will use the information to organize parent gatherings.
